1. ABOUT THIS PRIVACY POLICY. This Privacy Policy applies to the website owned and operated by Priority Concierge MD, P.L. (“Priority Concierge MD”). The “Site” means any website that Priority Concierge MD owns or controls. This Privacy Policy is intended to help you understand how Priority Concierge MD manages information about you.

By using the Site, you consent to Priority Concierge MD’s use of your information as described in this Privacy Policy. Priority Concierge MD reserves the right to change this Privacy Policy at any time without advance notice. Should any new policy go into effect, it will be posted on this site, and the revised Privacy Policy will apply only to information collected thereafter. You will be able to determine if this Privacy Policy has been changed since your previous visit by viewing the “Last Updated” information that appears at the top of this Privacy Policy. Your continued use of the Site will indicate acceptance of the changes. You may choose to opt-out by discontinuing your use of the Site. Please exit the Site immediately if you do not agree to the terms of this Privacy Policy or any revised policy.

2. PERSONALLY IDENTIFIABLE INFORMATION. While you may use Priority Concierge MD without providing personally identifiable information, some of the services on the Site require that you provide personally identifiable information. If you use the Site without providing personally identifiable information, the only information Priority Concierge MD collects will be non-personal information through the use of cookies or web beacons. If you choose to provide personally identifiable information to Priority Concierge MD for certain services, you are responsible for ensuring the accuracy of the personally identifiable information you submit to the Site. Inaccurate information will affect the information you receive when using the Site and Priority Concierge MD’s ability to contact you as described in this Privacy Policy. For example, your email address should be kept current because that it is how Priority Concierge MD communicates with you. Except as required by law or expressly authorized by you, Priority Concierge MD will not provide any personally identifiable information regarding you to any third parties. Any personally identifiable information that you submit to Priority Concierge MD will be used solely for the purpose of responding to inquiries or requests made by you.

As you use the Site, you will be given the option of receiving recurring informational newsletters via email from Priority Concierge MD. In order to subscribe to these newsletters via email, Priority Concierge MD will need your contact information, including name and email address. You can unsubscribe from the newsletters by simply following the directions included at the bottom of each email newsletter you receive from Priority Concierge MD.

If you have difficulties with the unsubscribe or subscribe service, please use the Customer Support Form to write and let Priority Concierge MD know about your problem. Priority Concierge MD Customer will manually unsubscribe you from receiving the newsletter within ten days.

Despite Priority Concierge MD’s efforts to protect your personally identifiable information, there is always some risk that an unauthorized third party may find a way around the Site’s security systems or that transmissions of your information over the Internet may be intercepted.

3. CHILDREN’S PRIVACY. Priority Concierge MD is committed to protecting the privacy of children. Neither Priority Concierge MD nor any of its services are designed or intended to attract children under the age of 13. Priority Concierge MD does not collect personally identifiable information from any person it actually knows is under the age of 13. The parent or guardian is solely responsible for providing supervision of the minor’s use of Priority Concierge MD. The parent or guardian assumes full responsibility for ensuring that any information submitted to Priority Concierge MD regarding a minor is kept secure and that the information submitted is accurate. The parent or guardian also assumes full responsibility for the interpretation and use of any information or suggestions provided through Priority Concierge MD for the minor.

4. NON-PERSONAL INFORMATION. Even if you do not provide personally identifiable information to the Site, Priority Concierge MD collects non-personal information about your use of the Site. Your visit to the Site is tracked through a standard web traffic statistics program, which keeps records of traffic on the site, as well as numerical counts of visitors by domain, URL, search engine, keywords used, and other standard web measurements. Priority Concierge MD’s server automatically collects data about your server’s Internet address when you visit the Site. This information, known as an Internet Protocol address, or IP Address, is a number that is automatically assigned to your computer by your Internet service provider whenever you are on the Internet. When you request pages from the Site, Priority Concierge MD’s servers may log your IP Address and sometimes your domain name. Your IP Address is used to help identify you and to gather demographic information about our customers as a whole, but does not include personally identifiable information. Our server may also record the referring page that linked you to us (e.g., another web site or a search engine); the pages you visit on this Site; the web site you visit after this Site; the ads you see and/or click on; other information about the type of web browser, computer, platform, related software and settings you are using; any search terms you have entered on this Site or a referral site; and other web usage activity and data logged by Priority Concierge MD’s web servers. Priority Concierge MD uses this information for internal system administration, to help diagnose problems with our server, and to administer the Site. Such information may also be used to gather demographic information, such as country of origin and Internet Service Provider. Priority Concierge MD may link this information with your personally identifiable information.

Priority Concierge MD may disclose user information to government authorities, and to other third parties when compelled to do so by government authorities, at its discretion, or otherwise as required or permitted by law, including but not limited to in response to court orders and subpoenas. Priority Concierge MD also may disclose user information in connection with investigations or when it has reason to believe that someone is causing injury to or interference with its rights or property, other users of the Site, or anyone else that could be harmed by such activities.

5. LINKS TO THIRD PARTY WEBSITES. The Priority Concierge MD Site may contain links to other websites. Once you enter another website (whether through an advertisement, service, or content link), be aware that Priority Concierge MD is not responsible for the privacy practices of these other sites. Priority Concierge MD encourages you to look for and review the privacy statements of each and every web site that you visit through a link or advertisement on Priority Concierge MD. Priority Concierge MD does not disclose your personally identifiable information to these third party websites.

In some cases, when you click on a link or an advertisement on the Site, in an e-mail or newsletter, your browser may be momentarily directed to the website of a third party which, acting on behalf of Priority Concierge MD, notes or “counts” your response to the e-mail or newsletter before re-directing your browser to your selected destination; this re-direction process may not be apparent to you.

6. INFORMATION YOU SEND TO PRIORITY CONCIERGE MD. This Privacy Policy does not apply to content, business information, ideas, concepts or inventions that you send to Priority Concierge MD by email or otherwise. If you want to keep content or business information, ideas, concepts or inventions private or proprietary, do not send them to Priority Concierge MD by email or any other method.

7. CHANGES TO YOUR PERSONALLY IDENTIFIABLE INFORMATION; COMPLAINTS. If you have a complaint or problem, please let Priority Concierge MD know by using the Contact Us link on the Site. If you want to (1) delete your personally identifiable information from Priority Concierge MD’s systems, (2) update the personally identifiable information that you have provided to Priority Concierge MD, or (3) change your preferences with respect to receiving newsletters or other activities, select use the Contact Us link on the Site. Upon your request, Priority Concierge MD will delete your personally identifiable information from its active databases and where feasible from its back-up media. You should be aware that it is not technologically possible to remove each and every record of the information you have provided to Priority Concierge MD from its servers.
